INSTRUCTIONS FOR FILLING UP THE ON-LINE APPLICATION FORM
Before proceeding to fill-up the On-Line Application Form, the candidates are required to read the following instructions given in two sections: SECTION-A and SECTION-B.

SECTION-A explains the steps needed to fill-up this form and SECTION-B illustrates the information for various fields of On-Line Application Form.


Step-1: Profile Registration
Please use this step to give general information about you to register the profile. The details of fields in the opened form have been illustrated in Section-B. After entering the required information in on-line form, submit the form by clicking on SUBMIT button. The subsequent screen displays a Admission Form No. and Login Id(which is actually the email which you entered during profile registration).A mail containing this information is also forwarded at your email-id.

Please note down the Admission Form No. carefully as you need to essentially mention it in all your future correspondence/queries related to your application form.



Step-2: Candidate Login
Use your login id and the password to login again to proceed further for Application Form filling. Use View/Edit Profile option to edit/correct your form (if needed) otherwise proceed to Step-3.


Step-3: Fill Registration Form
After entering the required information in appropriate Registration Form, click on SUBMIT button. Make payment form shall be displayed on subsequent screen and click on Make Payment button to deposit the fee.


Before proceeding for Step-4 or online payment please keep the following ready:

  1. Softcopy of your recent Photograph (3.5cm X 4.5cm) in JPG Format(less than 15 KB size)
  2. Scanned Signature (3.5cm X 1.5cm) in JPG (less than 15 KB size)



Step-4: Verify Payment
Use Candidate Login tab to enter in your login account and click on Verify Payment. Click on Verify Payment button. If your payment verified successfully, you will be prompted to upload softcopies of your recent photograph and scanned signature. After uploading of recent photograph and scanned signature, the final Confirmation Page of Application Form and Payment Receipt shall be generated. Take print out of this confirmation page and Payment Receipt in two copies (one for being sent to the University and other being retained by the candidate) and logout.

Please complete the printout of the Confirmation Page of Application Form by affixing recent photograph (same as the one uploaded in application form), put Signature and Left-Hand Thumb Impression of candidate and the Signature of parent/guardian at appropriate places. The duly complete copy of the Confirmation Page of Application Form should be sent along with copy of Payment Receipt to The Coordinator, Admissions-2017, Madan Mohan Malaviya University of Technology, Deoria Road, Gorakhpur-273010 (U.P.) so as to reach latest by 5 P.M. April 17, 2017 .

If the payment of the fee is made through online (E-Payment) and due to some difficulties the candidate is not sure weather the payment is processed or not, then please use login tab to enter in your login account and use verify payment option to check your payment is successful or not.



Please do keep 6 additional copies of the recent photograph (same as uploaded on ON-Line Application Form) for future use.

Details of Fields of On-Line Application Form
  1. Seeking Admission for
    Choose your course group.
  2. Candidate Name
    Write your FULL NAME (special characters/abbreviation not allowed) as recorded in your High School or equivalent examination certificate. Leave one space blank between any two parts of the name. Do not use prefix such as Shri/Mr./Km./Ms. etc. with your name.
  3. Father Name
    Write FULL NAME (special characters/abbreviation not allowed) as recorded in your High School or equivalent examination certificate. Leave a blank space between any two parts of the name. Do not use prefix such as Shri/Mr./Dr./ Late etc. with the father's name.
  4. Mother Name
    Write FULL NAME (special characters/abbreviation not allowed) as recorded in your High School or equivalent examination certificate. Leave a blank space between any two parts of the name. Do not overshoot the boxes. Do not use prefix such as Shri/Mr./Dr./ Late etc. with the mother's name.
  5. Date of Birth
    Write your date of birth exactly as recorded in your High School or equivalent examination certificate.
  6. Gender
    Select the sex as applicable
  7. Category
    Select the category as applicable out of UPGE/ UPSC/UPST/UPBC/ UPGD/ GDSC/GDST/GDBC/GDDA.

    Category Definitions

    (a) Candidates, who have passed the qualifying examination from an institution located in the State of Uttar Pradesh and who do not claim for any reserved category are eligible for admission to all the courses in the University against available General (OPEN) seats (No certificate required).

    (b) Candidates, who have passed the qualifying examination from an institution located in State of U.P. and are domicile of U.P.; and belong to Schedule Caste (SC) of U.P./Schedule Tribe (ST) of U.P./Other Backward Classes of U.P. are eligible for admission against available reserved seats of their category. Certificate no. 1 or 2 as applicable is required. List of SC, ST and OBC castes as per SC/ST Act 1976 and Government Order 4/1/2001-TC/Ka-2/2002 dt. 19/09/2002 is attached in Appendix E of Information Brochure.

    (c) Candidates, who have passed the qualifying examination from an institution located outside U.P. and whose parents are domicile of U.P.; and who have not claimed for any reserved category, are also eligible for admission to courses offered in the University under General (OPEN) category. Such candidate has to produce the domicile certificate of his/her parents (Father OR Mother only) at the time of counselling (certificate no. 3 in Appendix C of Information Brochure).

    (d) Candidates, who have passed the qualifying examination from an institution located outside U.P.; and whose parents are domicile of U.P.; and who belong to Schedule Caste of U.P./Schedule Tribe of U.P./Other Backward classes of U.P. are also eligible for admission against reserved seats of their category. Such candidate has to produce the domicile certificate of his/her parents (Father OR Mother only) (Certificate no. 3) and category certificate (Certificate no. 1 or 2 as applicable) at the time of counselling. List of SC, ST and OBC castes as per SC/ST Act 1976 and Government Order 4/1/2001-TC/Ka-2/2002 dt. 19/09/2002 is attached in Appendix E of Information Brochure.

    (e) The domicile requirement to son/daughter of following is relaxed and admission is permitted to such:
    1. Defence personnel settled in U.P. on the date of Admission Test after retirement/being disabled in action or Defence personnel killed in action and the dependents settled in U.P. on the date of Admission Test. Certificate no. 5 Appendix C of Information Brochure is required.
    2. Defense personnel who are not domicile of U.P., but are posted in U.P. on the date of Entrance Examination. Certificate no. 5 in Appendix C of Information Brochure is required.
    3. Employee of All India Services belonging to U. P. cadre. Certificate no. 11 in Appendix C of Information Brochure is required.
      GDDA candidates will be considered as UPGD category for all other benfits.

    Vertical Reservation (Category)

  8. Subcategory
    Select the sub-category as applicable out of UPFF/UPAF/UPHC. All female candidates will be eligible for UPGL and may opt for one more sub category out of UPFF/UPAF/UPHC.
    • The candidates (except girls, who can opt for any one UPFF/UPAF/UPHC along with UPGL) can be given only one type of horizontal reservation. The eligible candidate can opt for only one type of horizontal reservation out of UPFF/UPAF/ UPHC/UPGL. In case the candidate does not opt for any one of the four suitable horizontal reservations, horizontal reservation shall not be admissible after the examination.
    • Horizontal Reservation (Sub-category in each vertical reservation category): Not Applicable for M.Tech./Ph.D.

    • The candidates can claim only one type of horizontal reservation out of UPFF/UPAF/UPHC. However, the girl candidates can also claim for any one of UPFF/UPAF/UPHC alongwith UPGL.
    • Subsequent request for the change of category/sub-category filled in OMR application form or ON-Line submission shall not be entertained under any circumstances.
    • Category/sub-category must be supported by the relevant certificates as per proforma given in the Appendix-C of Information Brochure to be produced at the time of document verification in counseling, failing which the candidates shall be treated in General Category.

  9. Religion
    Select as applicable form the drop down list.
  10. Handicapped/Type of Handicap
    The candidates opting for UPHC category will have to select as applicable out of Type-I/Type-II/Type-III
    Refer the following table for details.
    Physically Handicapped/Disabled
    Type – I Minimum 40% permanent Visual impairment
    Type – II Minimum 40% permanent Locomotors disability
    Type – III Minimum 40% permanent speech and Hearing impairment
  11. Rural Weightage Claim
    Select as applicable out of Rural/Urban.
    • A weightage of 3% of the total marks obtained at Admission Test-2014 will be given to such UPGE candidates for B.Tech. course, who have passed the qualifying examination from an institution situated in rural area of Uttar Pradesh.
    • For this purpose, the institutions should be situated in the jurisdiction of Gram Panchayat but the area should not be industrially developed area. Such candidates must produce the certificate number 7 at the time of document verification.
    • Rural weightage is not applicable for a candidate opting for any other reservation and for a candidate opting direct admission to second year.
    • Rural weightage is not applicable in MBA and MCA courses.
    • Rural weightage is not applicable for candidates under UPGD category.
  12. Nationality
    Select as applicable form the drop down list.
  13. Mother tongue
    Select as applicable form the drop down list.
  14. Postal address, City, State, Pin code
    Write your correspondence address along with city, state and pin code in different lines as applicable.
  15. Mobile number
    Write a valid ten digit mobile number so that you may be contacted upon requirement.
  16. Email-id
    Write correct e-mail address. Please note that all correspondences will be made on it.
  17. Password
    Enter your password minimum 8 characters and maximum 10 characters atleast 1 alphabet and 1 number.
  18. Parent’s Income
    Select the income slab (total income from all sources) as applicable.
